Background checks often include criminal records, educational milestones, employment and credit history, driving records, and even social media.

Mental Health Screening
A mental health screening may be conducted by the surrogacy agency or medical clinic to ensure that each intended parent involved is capable and willing to be a parent.

Pregnancy Preferences
This section of the form will ask if you are willing to carry multiples (twins or more). We will also go over your communication preferences with the intended parents. Additional specific preferences may also be originated by the surrogate here.
Background Checks
