Then, you will begin to fill out the application forms! This part will consist of filling out your:

- Health history, including diagnosed reproductive issues and any complications with past births
- Lifestyle choices, like alcohol consumption, smoking habits, dietary restrictions, drug usage (recreational and prescription), exercise routine, place of employment, scheduling parameters, and living situation
- Mental and emotional health history
Once the application is completed, we will discuss your preferences going forward with your surrogacy journey.
